TERMS OF SERVICE & WEBSITE DISCLAIMER

Welcome to the website of Rockdale Mental Health, PLLC (“Rockdale Mental Health,” “we,” “our,” or “us”). These Terms of Service govern your use of rockdalementalhealth.com and related website features.

By accessing or using this website, you agree to these Terms. If you do not agree, please do not use the website.

 

1. INFORMATIONAL PURPOSE ONLY

The information on this website is provided for general educational and informational purposes. It is not medical or psychiatric advice and is not a substitute for evaluation, diagnosis, treatment, or emergency care from a qualified health professional.

Do not start, stop, or change any medication or treatment based solely on information found on this website.

 

2. NO PROVIDER–PATIENT RELATIONSHIP

Viewing this website, using the fit guide, submitting a general inquiry, or requesting an appointment does not by itself create a provider–patient relationship.

A provider–patient relationship is established only after Rockdale Mental Health has accepted you for care and all required intake, consent, identity-verification, and clinical steps have been completed.

Submitting an appointment request does not guarantee acceptance, treatment, medication, or a particular appointment time.

 

3. EMERGENCIES AND URGENT NEEDS

Rockdale Mental Health is not an emergency or crisis service. This website, email, text messaging, contact forms, and the client portal may not be monitored continuously.

If you are experiencing a medical or mental-health emergency, believe you may harm yourself or someone else, or need immediate assistance:

• Call 911;
• Call or text 988; or
• Go to the nearest emergency department.

Do not use this website or a routine appointment request for urgent or emergency needs.

 

4. SERVICES AND ELIGIBILITY

Rockdale Mental Health provides outpatient psychiatric evaluation, medication management, and related telehealth services.

Services are generally available to patients age 17 and older who are physically located in Washington at the time of each appointment. Eligibility may also depend on clinical appropriateness, consent requirements, licensing rules, insurance requirements, and other applicable laws or policies.

Rockdale Mental Health does not currently provide in-person care through this website.

We may decline, postpone, or discontinue services when clinically appropriate, when required by law, when required information is not provided, when telehealth is not appropriate, or when a higher level of care is needed.

 

5. TELEHEALTH

Telehealth has benefits and limitations. Potential limitations include technology failures, interruptions, reduced ability to perform a physical examination, privacy risks, and the possibility that telehealth may not be appropriate for every condition.

You are responsible for:

• Being physically located in Washington during each appointment;
• Providing an accurate physical location and emergency contact information;
• Participating from a reasonably private and safe setting;
• Using a reliable internet connection and compatible device; and
• Not driving or operating machinery during an appointment.

If technology fails, Rockdale Mental Health may attempt to reconnect, contact you by another approved method, or reschedule the appointment.

 

6. PRESCRIBING AND CONTROLLED MEDICATIONS

Scheduling an evaluation does not guarantee that any medication will be prescribed.

All prescribing decisions are based on clinical judgment, applicable law, professional standards, medical history, safety considerations, monitoring requirements, and the information available at the time of care.

Rockdale Mental Health may require records, laboratory testing, vital signs, pharmacy information, coordination with other clinicians, in-person evaluation by another provider, or more frequent follow-up before prescribing or continuing a medication.

Controlled medications are never guaranteed and may be declined or discontinued when clinically appropriate or legally required.

 

7. INSURANCE AND SELF-PAY

Rockdale Mental Health participates with certain commercial insurance plans and also offers self-pay services.

Insurance participation does not guarantee that every plan, employer group, product, or service is covered. Network status, eligibility, prior authorization, copays, coinsurance, deductibles, and other patient responsibility vary by plan.

You are responsible for confirming coverage and for paying amounts assigned to you by your insurer, including services your plan denies or determines are not covered, except where prohibited by law or contract.

Current self-pay rates are:

• New-patient psychiatric evaluation: $200
• Follow-up appointment: $90

Rates may change. The rate and financial terms provided during scheduling or in your financial agreement control if they differ from information displayed on the website.

Cancellation, late-cancellation, no-show, payment, refund, and collections terms are governed by Rockdale Mental Health’s separate financial and practice policies.

 

8. APPOINTMENT REQUESTS AND CLIENT PORTAL

Appointment requests, intake forms, telehealth visits, secure messages, clinical documents, and certain payments are handled through the secure client portal and related service providers.

You are responsible for providing accurate and complete information and for protecting your portal credentials.

Do not share your password or allow another person to access your account. Notify Rockdale Mental Health promptly if you believe your account has been compromised.

 

9. WEBSITE CONTACT FORMS, EMAIL, AND TEXTING

General website forms, ordinary email, and standard text messaging may not be secure. Do not use them to submit detailed medical history, medication information, sensitive records, or urgent clinical concerns.

Use the secure client portal for clinical communications whenever available.

By providing a phone number or email address, you authorize Rockdale Mental Health to respond using that method, subject to your communication preferences and applicable law. Standard carrier or data charges may apply.

 

10. WEBSITE TOOLS AND FIT GUIDE

Interactive website tools, including the fit guide, provide general practice information only. They do not diagnose a condition, determine medical necessity, establish eligibility, verify insurance benefits, or replace professional evaluation.

Unless a tool expressly states otherwise, responses entered into an interactive website guide are not intended to become part of your medical record.

 

11. ACCEPTABLE USE

You agree not to:

• Use the website for unlawful, fraudulent, abusive, or harmful purposes;
• Attempt to gain unauthorized access to the website, client portal, or related systems;
• Introduce malware, automated scraping tools, or code that interferes with site operation;
• Impersonate another person;
• Submit false or misleading information; or
• Copy, reproduce, or commercially exploit website content without permission.

We may restrict access when we reasonably believe these Terms have been violated.

This website may link to or use third-party services, including website hosting, scheduling, telehealth, payment, insurance, analytics, referral, and educational resources.

Third-party services have their own terms and privacy practices. Rockdale Mental Health is not responsible for the availability, content, security, or privacy practices of a third-party website or service that we do not control.

 

14. WEBSITE AVAILABILITY AND ACCURACY

We make reasonable efforts to keep the website accurate and available, but information may become outdated and the website may experience interruptions, errors, or technical problems.

The website is provided on an “as available” basis. To the fullest extent permitted by law, we disclaim warranties regarding website availability, accuracy, completeness, or fitness for a particular purpose.

Nothing in this section limits duties that cannot legally be waived in connection with professional health-care services.

 

15. LIMITATION OF LIABILITY

To the fullest extent permitted by law, Rockdale Mental Health will not be liable for indirect, incidental, special, consequential, or punitive damages arising solely from use of, inability to use, or reliance on this website or third-party links.

This limitation applies to website use and does not limit any rights or remedies that cannot be waived under applicable health-care, consumer-protection, privacy, or professional-liability laws.

Website information is handled as described in our Website Privacy Policy. Protected health information created or maintained as part of clinical care is handled under our Notice of Privacy Practices and applicable federal and Washington law.

We may update these Terms to reflect changes in law, services, technology, or business practices. The updated version will be posted with a revised effective date.

Your continued use of the website after an update constitutes acceptance of the revised Terms.

 

18. GOVERNING LAW

These Terms are governed by the laws of the State of Washington, without regard to conflict-of-law principles, except where federal law or another applicable law controls.

Any legal venue provision is subject to rights and protections that cannot be waived under applicable law.
